Student Registration
Students who have been determined to be legal residents, or those who temporarily reside within the district as homeless children in accordance with the Commissioner's Regulations, and who meet district registration requirements, are entitled to attend the Public Schools within the East Meadow School District. In accordance with its obligation, the East Meadow Board of Education has adopted this policy in order to regulate student enrollment, to maintain the quality of its educational programs for district resident children, and in order to ensure that its educational programs and resources remain available for only those students who are legally entitled to benefit from them. When there is cause to suspect that a student attending or sought to be enrolled in the East Meadow School District is not eligible to attend the District's schools, the District will investigate the student's eligibility. Community members with information regarding students who may be ineligible to attend District schools are encouraged to contact the Registration Office at (516) 478-5526. Calls will be confidential and can be made anonymously. Students found to be ineligible will be excluded from continued attendance or enrollment pursuant to the Policy and Regulations of the Board of Education and the laws of New York State
How to register your child for attendance in an East Meadow School
What does my child need in order to attend an East Meadow school?
Please fill out registration packet appropriate for your child. One packet per child.
- REGISTRATION PACKET - FOR EAST MEADOW UNION FREE PUBLIC SCHOOL--GRADES PRE-K, K-12. Requires residency, birth certificate and immunizations with physical form.
- REGISTRATION PACKET - FOR EVALUATION AND TRANSITIONING PRE-SCHOOL AGE CHILDREN (2,3,4 YEARS OLD) FOR SPECIAL EDUCATIONS SERVICES ONLY. Requires residency, birth certificate and immunizations only.
- REGISTRATION PACKET - FOR ALL STUDENTS ATTENDING NON-EAST MEADOW PUBLIC SCHOOL PROGRAMS--GRADES K-12. Requires residency and birth certificate.
Proof of Immunization:
New York State Public Health Law mandates that schools shall not permit a child to be admitted unless the parent provides the school with a certificate of immunization or proof from a physician, nurse practitioner or physician’s assistant that the child is in the process of receiving the required immunizations. Please have your physician complete the health form and bring it with you to registration. Clinic records or baby shot books, signed by a physician, are also acceptable proof of immunization. Feel free to contact the school nurse with any questions you may have.

- 3 OPV polio or 4 IPV polio
- 3 Hep B
- 2 Measles
- 1 Mumps
- 1 Rubella
- 1 Varicella
- Lead Screening for Kindergarten
- Physical exam within year of entrance
As part of your child’s requirements for school, a physical examination has been required for students in Pre-Kindergarten, Kindergarten and in Grades 2, 4, 7 and 10. The physical form must be dated, signed and stamped by a physician.
A LAW WAS RECENTLY ENACTED THAT EXPANDS HEALTH SCREENINGS TO INCLUDE THE DENTAL HEALTH OF STUDENTS IN NEW YORK STATE.
When we require that your child have a physical examination, we will be requesting a dental certificate as well. Please have your family dentist complete the dental certificate and return it to the appropriate school nurse. Please call the health office if you have any health related questions.

PLEASE NOTE:
You are required to change your address with the school district as soon as your change in residency has been made. Neglect in address changes may result in district residency audits being conducted. Please follow the proof of residency requirements for all address changes.
